When completing Form 33-109F4, users will need to specify an individual’s proficiency qualifications. You will need to know student numbers for individuals who have completed certain educational and career development programs, and in some cases the registrant may not have this information handy. For convenience we list below the following information:

• Association for Investment Management and Research (AIMR)
  Please contact AMIR Member Services at 1-800-247-8132, or by sending an email to info@aimr.org. In the e-mail, individuals must provide unique identification information (e.g. home or business address) in order for the AMIR representative to confirm their identify.
• Canadian Securities Institute (CSI)
  Please contact the Canadian Securities Institute (CSI) at 416-364-9130 or toll free at 1-866-866- 2601 (English) or 1-866-866-2602 (French).
• Investment Funds Institute of Canada (IFIC)
  In order to obtain their student numbers, individuals should contact IFIC at the following e-mail address: ifse@ifse.ca. Individuals will be required to provide IFIC with unique identifying information (e.g. a Social Insurance Number) in order for the IFIC representative to confirm his or her identity.
• Institute of Canadian Bankers (ICB)
  In order to obtain their student numbers, individuals should contact ICB Student Services at 1-800-361-7339.
